Front cheek filler / tear trough filler

Front cheek filler is a very important treatment for beautification and also for antiageing purposes. The lid cheek junction is one of the most important areas on the face with ageing. It is the area where the lower eyelid meets the cheek, which is crossed by the tear trough.

Transitional areas like this one often reveal ageing as it appears. When patients are young, they have a smooth transition from the lower lid to the cheek.

As we age, with the fat and bone loss in the area, the under eye area hollows and the tear trough lengthens.

Before injecting the troughs or the cheeks, the most important part of the examination is the assessment.

The Assessment

What is going on with the patient’s anatomy?

What are their cheeks like? Are they generally deflated, or are they missing volume anteriorly (at the front) or laterally (the sides).

Does the front cheek have a nice convex and young appearing surface, or is it flat or worse, convex or depressed?

For tear trough results to be good and smooth and natural, the troughs MUST have good support from the cheek. There is no point in attempting to fill a trough that has no support under it, it will just look aesthetically unpleasant.

Here we have a case where the patient had lost some volume in her front cheeks. Here we show the improvement after one side’s front cheek has been treated.

As you can see, supporting the tear trough area by injecting the deficient front cheek can shorten the tear trough, which is what has happened in this case, and why the patient looks so much better on the treated side.
